Our in-house social worker(s) empower patients to live their best lives by connecting people to community resources. We help people of all ages and backgrounds improve their quality of life through assisting each patient to access programs available within their community.
Examples include but are not limited to employment, financials, food, housing, social connectedness, and transportation.
Financial Advocate
Community Health Center of Fort Dodge has a trained Certified Application Counselor to review your health insurance needs. Our insurance enrollment services are free and open to the general public as well as CHC patients. Our Fort Dodge Financial Advocate is a SHIPP Certified Counselor. SHIPP which stands for Senior Health Insurance Information Program is a free, confidential service of the State of Iowa Insurance Division. We are able to assist patients and non-patients of the Community Health Center of Fort Dodge with Medicare open enrollment including Medicare Part D enrollment. In addition, our Financial Advocate can assist you with setting up a payment plan for your healthcare services. Anyone can make an appointment or walk in during office hours to receive outreach assistance. To make an appointment call 515-576-6500. This assistance is free of charge.
Interpretation and Translation Services
We have many interpreters on staff to bridge the gap of language barriers for patients being seen by a provider or in need of information on the phone. Many of our printed materials are also available in multiple languages.
